SYDNEY: The human toll of the tragic July 2023 Taipan helicopter crash was starkly outlined as families of the four deceased airmen spoke publicly for the first time, seeking answers and accountability from the Defence Department for the catastrophic loss.
Caitland Lyon described the “ruining” sound of her five-year-old son’s scream after she told him his father, Captain Danniel Lyon, would not be coming home. Alongside the families of Lieutenant Max Nugent, Corporal Alex Naggs, and Warrant Officer Phillip Laycock, she insists the deaths were preventable, pointing to a persistent culture that prioritizes risk over safety.
The Defence Flight Safety Bureau (DFSB) released its Aviation Safety Investigation Report in May, concluding that the primary cause of the accident was an unrecognised loss of spatial orientation—a state where a pilot loses awareness of their aircraft’s position relative to the earth.
The report, which made 46 recommendations to improve safety, found that it was “unlikely” that the TopOwl 5.1 helmet-mounted display—which had “known hazards”—was the contributing factor. The Chief of Army, Lieutenant General Simon Stuart, has stated that Defence accepts and is implementing all recommendations.However, this official conclusion is vehemently contested by both the families and a former military insider.
Retired test pilot Major Ian Wilson testified at the ongoing inquiry that he had previously warned senior officers that the TopOwl 5.1 system was dangerously flawed. Wilson’s 2020 report documented severe errors, including the display of incorrect flight information (pitch shown as bank) and a 50 per cent loss of night vision efficiency, leading him to warn of a “substantial risk of multiple deaths.”
The airmen’s loved ones support Wilson’s account, chillingly recalling that both Captain Lyon and Lieutenant Nugent had spoken to them about the poor night vision capability of the TopOwl system long before the crash.
“In what world do you give someone a piece of equipment that is full of errors and go, ‘That’s okay. They’ll be right’?” asked Caitland Lyon.
Major Wilson alleges that the DFSB report is an attempt to conceal the truth, claiming that senior officers who forced the introduction of the flawed system later exerted a “disproportionate influence” over the accident investigation to clear their own decisions. He called it “unconscionable” to place all the blame on the deceased pilot without scrutinizing the mandatory equipment.
The Government awaits the findings of all investigations into the accident, echoing the families’ desire for a full understanding of the tragic loss.
